Photography Permissions
Your child may be photographed for church participation, social media, volunteer training, etc.
Pick-Up & Security Policy
Participants receive a name tag sticker; the parent gets a matching security tag (must be shown at pick-up). Only 18+ adults may pick up.
By submitting this form, I agree my participant may not be dropped off; a parent/guardian must remain on campus during Sunday/Tuesday activities (except designated drop-off events). I confirm this form is accurate to the best of my knowledge.
